    Confidentiality and Release of Information

    Franciscan University of Steubenville recognizes that student disability records contain confidential information and are to be treated as such. All disability related information submitted for the purpose of receiving accommodations is housed in the office of the Director of Student Academic Support Services (SASS) and is considered part of the student’s educational record. Each student has a separate file in a secure filing cabinet and only SASS staff has access.

    Information related to a disability may be disclosed only with the permission of the student or as permitted by the university’s student records policy and federal law.

    The SASS staff is very sensitive to and respectful of your right to privacy and confidentiality.

    • SASS views all materials pertaining to a student’s disability as confidential. This policy is based upon government mandates regarding the confidential treatment of disability-related information.
    • Disability information may be released only when a student has signed a Release of Information form.
    • The Family Educational Rights and Privacy Act of 1974 (FERPA), also known as the Buckley Amendment, and the Americans with Disabilities Act Amendments Act (ADAAA), do not allow faculty access to disability-related information.
    • According to the Association on Higher Education and Disabilities (AHEAD), “It is only necessary to share with the family the information that a student has a documented disability and need for accommodation(s).” Faculty members have no need to know the nature of the disability, “only that it has been appropriately verified by the staff assigned this responsibility on behalf of the institution.”
    • A student may request to review the contents of his/her own file when the SASS Director is present. However, all information in the file is the property of FUS.
    • A student may request in writing his/her medical documentation upon leaving the University. Otherwise, it is stored for three years following the last enrolled date and then destroyed.

    Release Forms

    • Release of Information to SASS

      This form is used to request documentation of a disability from a medical or counseling facility where the disability was diagnosed.
    • Release of Information from SASS 

      This form is used when a student is requesting information from their file be sent to a testing agency, medical, educational professional, or to the student directly.
