Enrolling Your Spouse in Lifestyle Returns

If employee is a Highmark member

1. Go to the Highmark page and log in.
2. Choose the Member tab
3. Choose "Register Now"
4. Page titled: Registration
5. Enter the Highmark card holder's Member Identification number
6. Fill everything else as the spouse's information.

If employee is not a Highmark member

Get a form from Human Resources to enroll as an Associate Member. Once you receive the mailed ID card go online and use the instructions outlined above.
